Nobody Read the PRD: AI's New Product Failure Mode

Non-technical stakeholders are now generating product specifications faster than they can read them. They iterate with AI for weeks, anchor to artifacts they have only scanned (without sharing beyond their AI), and arrive at scoping conversations convinced they have designed a product they have never actually examined. The agency team is no longer partnering with the client. They are partnering with the client's AI assistant, and the client trusts the AI more than the team they hired.

Talk to any product lead at an agency right now and a version of this story is happening to them.A non-technical stakeholder shows up to a sprint planning meeting with a 200-page specification. Maybe more. The document is comprehensive, professionally formatted, full of feature descriptions written in confident product language. The stakeholder wants the team to review it, integrate it, and build everything in it. Within ten minutes of conversation, the product lead realizes something is wrong. The stakeholder cannot answer specific questions about what is in the document. They cannot explain why a particular feature exists or what user problem it solves. They cannot rank priorities. When pressed on contradictions between sections, they get defensive, then vague.They have not read the document, well not completely.They generated it, scanned it, watched it grow over weeks of iteration with their AI tool of choice, and arrived believing the artifact represents their thinking. The harsh reality, it does not. It represents their prompting.This is the failure mode nobody has named yet. It is happening across the industry and it is going to get worse before anyone admits it is happening. I want to describe it in plain terms, explain why it is structurally inevitable, and offer three early moves product leaders can make to reduce the damage.The Labor That Creates the ConvictionThe first thing to understand is that the stakeholder is not lazy. They have done real work. They have spent weeks iterating. They have rewritten sections, asked follow-up questions, requested deeper analysis, and pushed their AI assistant to refine and expand. Every cycle felt like rigor. Every revision felt like progress. By the time they arrive in front of the team, they have hundreds of hours of perceived effort behind the artifact.This is the cruelty of the dynamic. Real labor produced an unread artifact, and the labor itself created a deep conviction that the artifact is correct. The stakeholder is not pretending to have done the work. They have done the work. The work just produced something different from what they think it produced.In traditional product practice, the act of writing a specification was the act of thinking through the product. A 2023 study of medical residents found that documentation was not a separate task from clinical reasoning. The writing process itself was how the residents developed their understanding of patient cases. They could not have produced the document without first understanding the case, and they could not fully understand the case without writing it down. The artifact and the thinking were the same thing.AI breaks that loop. The artifact appears without the thinking. And because the stakeholder was present for every prompt, every revision, every iteration, they experience the document as something they authored. They did not. They supervised it. There is a difference, and the difference is the entire problem.The Trust InversionHere is what I think is actually going on, and I have not seen it named clearly anywhere.We are not partnering with our clients anymore. We are partnering with our clients' AI assistants. And the client trusts the AI more than they trust the team they hired.Sit with that for a moment. The team has the contract. The team has the user research. The team knows what stage the build is in, what is technically feasible, what is in scope, what was decided in the last six discovery sessions. The AI has none of this. The AI has whatever the client typed into the prompt, plus its training data, plus its inclination to produce confident output regardless of how grounded that output actually is.And yet the AI feels more authoritative to the client than the team does. It is faster. It never says no. It never asks an inconvenient question about budget or timeline. It produces comprehensive documents on demand. It validates the client's instincts and expands on them with impressive vocabulary. The team, by comparison, pushes back. The team raises constraints. The team takes time. The team is, frankly, less satisfying to work with than the AI.So the client's primary product collaborator is now their AI. The agency has been demoted to executor of decisions made between the client and a model that has never seen the contract.This is the structural shift. Everything else flows from it.Three Forces StackingOnce you see the trust inversion, the three failure modes that follow become predictable.Force one: AI produces without prioritization. The Standish Group has reported for decades that the top factor behind project failures is requirements problems. The classic version of that problem was incomplete or ambiguous requirements. The 2026 version is the opposite: requirements that are too complete, too detailed, and entirely unprioritized. AI does not know what stage the build is in or what the contract scopes. It does not distinguish MVP features from future-state ones. Every feature ships with the same authoritative tone, in the same document section, with the same level of detail. To the reader, they all look like requirements.The result is that the stakeholder receives what amounts to a multimillion-dollar app plan and treats it as the agreed scope. Their AI never told them that 80% of those features should not exist in the initial release. Why would it? They never asked.Force two: volume outpaces review capacity. Vectara research has found generative AI tools hallucinate between 2.5% and 22.4% of the time depending on the model and use case. Some hallucinations are factual. Many can be subtle. A feature that sounds plausible but does not fit the product. A capability that contradicts an earlier section. A user flow that requires infrastructure the team has not built and was never asked to build. The stakeholder cannot catch these because they are no longer reading the document at human speed. They are generating faster than they can review, and as the volume grows, their review degrades to scanning. They pick up on what sounds impressive and approve what reads as comprehensive. They do not notice the contradictions or the impossibilities because catching them would require the kind of close reading that AI has trained them out of doing.A 2025 ScienceDirect study examined how AI usage affects users' ability to evaluate their own work. Participants using AI on logical reasoning tasks improved their performance by three points compared to a baseline population. They overestimated their own performance by four points. The gap between actual capability and perceived capability widened with AI use, not narrowed. More striking, higher AI literacy correlated with worse self-assessment accuracy. The people most confident they were using AI well were the worst at judging their own output.This is the empirical foundation for what every product team is now experiencing in client meetings. Stakeholders genuinely believe their AI-assisted work is better than it is, and the more comfortable they get with the tools, the wider the gap grows.Force three: anchoring without comprehension. The stakeholder ends up anchored to features they cannot describe. They picked them by name appeal, by impressive description, by what scanned well during their review. Ask them what a particular feature does and they will read the description back to you, in the AI's language, not their own. Ask them why it matters and they will struggle. Ask them what would happen if you removed it and they will resist on principle, because removing it feels like losing something they committed to, even though they cannot articulate what.This is sunk cost compounded by AI-generated false rigor. The stakeholder has spent weeks building a relationship with their own product idea. The relationship is real, but the product idea is not feasible.What It Looks Like DownstreamThe pattern is consistent enough now that you can spot it within one or two meetings. Here are the signals I look for:The stakeholder uses vocabulary in writing that they do not use in conversation. Specs grow between meetings without commensurate engagement on their part. Feature lists arrive without priority distinction. When asked which feature is most important, they list seven. When asked what the product must do at launch, they name the entire roadmap. When asked to remove anything, they resist across the board. When pressed on a specific feature, they defend the artifact as a whole rather than the feature itself.Internal contradictions show up in the document and the stakeholder has not noticed them. A user flow on page 47 conflicts with a permissions model on page 112. A data assumption in the architecture section contradicts a reporting requirement three sections later. These are the fingerprints of generation without review. Humans who write specs tend to catch their own contradictions because the act of writing forces consistency. Humans who supervise AI specs do not, because the AI does not know it is contradicting itself across a 200-page document.Downstream, the team starts hemorrhaging hours on triage. Every sprint, someone has to read the latest revision, identify what changed, flag what is new, and translate it into engineering-actionable input. This work was not scoped or budgeted. It is invisible to the client, who experiences their revisions as productivity. To the team, the revisions are a tax on every other piece of work the team is supposed to be doing.Trust in the team erodes. People start discounting everything from the client, including the things they actually thought through. The senior engineer who used to give the client the benefit of the doubt stops doing so. The Product Manager who used to advocate for the client's vision starts advocating for the team's sanity. By the time anyone names the dynamic out loud, the relationship is already in a different place than it was at kickoff.Why This Is a 2026 ProblemA year ago, this pattern was rare. A handful of clients were experimenting with AI for product documentation, and most of them produced obvious AI artifacts that everyone could identify and discount.In 2026, three things changed at once.AI accessibility crossed a threshold where nearly every founder has and AI tool in their workflow.Output quality improved enough that AI-generated artifacts now look professional, organized, and authoritative on first read.Non-technical adoption matured to the point that founders who spent months iterating with AI now believe they are doing rigorous product work, because the experience of doing it feels rigorous.The pattern was not invisible last year, however it was rare. It is becoming standard now. Every agency I know is dealing with at least one version of it. Most of them have not put a name on it yet, which is part of why the response has been so slow.How to Recognize It in the First ConversationHere is the diagnostic I am experimenting with now, in the first or second conversation with a new stakeholder:I ask them to describe the product in their own words, without referring to any document. Describe the product as if you are explaining it to a friend who has never heard of it. The stakeholders who have done real product thinking can do this in two or three minutes. The stakeholders who are anchored to an unread AI artifact cannot, they don't have the elevator pitch prepared because they didn't internalize it. They quote phrases that sound rehearsed. They get vague when asked follow-up questions about why specific features matter.I ask them to name the one user this product is for. If they cannot, the product has not been thought through. If they can, I ask what that user is doing right now without this product, and what changes for them after launch. This filters out feature-level thinking and forces outcome-level thinking. The stakeholders who have spent time thinking through the "why," know who they are targeting with their solution. The ones whose AI did the work for them will not be so sure.I ask them what they would cut. In a review of every feature in the document, which one could you remove and still ship a product worth shipping? The honest answer is usually most of them, and the stakeholders who have done the prioritization work know that and understand why each feature is proposed. The ones who have not will resist the question itself, because the question presumes the document is not already prioritized, and admitting that would unravel the conviction the document has built.These three questions take 15 minutes and surface the dynamic before it consumes the engagement.Reducing the Sunk Cost: Three Moves for Product LeadersRecognizing the pattern is the easy part. Doing something about it without destroying the client relationship is harder. Here are three moves I am beginning to test in my process.Treat the client's AI as a stakeholder. Not as a too they employ, but as a true additional stakeholder. Surface it in conversation. Ask which model the client is using. Ask what context they are feeding it. Ask to see their prompts when relevant. Acknowledge openly that the AI is shaping the product alongside the team. This sounds confrontational and it is not. It is honest. The AI is already a stakeholder in the project, with influence over scope and direction. The only question is whether it operates in shadow or in the open. A stakeholder you can see is a stakeholder you can manage. Once the AI is on the table as a participant with limitations, the team can start having productive conversations about what the AI knows and does not know, and the trust inversion starts to correct itself.Run a North Star exercise before the specification exists. This is the most important thing on this list, and I think it is becoming non-optional. One user. One outcome. One launch-critical capability. What outcome drives the entire solution forward.Force the prioritization conversation early, before the client has anchored on a sprawling document. The North Star becomes the reference point that every future feature gets ranked against. When the spec inevitably tries to expand, the team has a defensible basis for pushing back: does this feature serve the North Star outcome, or does it serve a future-state vision the AI surfaced without context? Outcomes do not bloat, but specs do. Pre-2024, missing this exercise meant a suboptimal product. Now, it means the AI fills the strategic vacuum and the client anchors to whatever the AI produces. The cost of skipping it has gone up by an order of magnitude. We treat the North Star exercise as the first thing that happens with any client, before any feature conversation. It used to be best practice. Now, its becoming a survival tactic.Make the team's context visible. The trust inversion happens because the AI feels comprehensive and the team feels like execution. The fix is to reverse the visibility. Document what the team knows that the AI does not. The build stage, technical constraints, contract scope, prior decisions made in discovery, user research, integration realities, etc. When the AI's output conflicts with the team's context, the team has a documented basis for pushing back, and the client has a tangible reason to weight the team's input above the AI's. You are not arguing the AI is wrong, rather you are showing that the AI is operating with a smaller context window than the people the client hired. The re-positioned framing is posied to win, because it is true.These three moves do not solve the problem. They reduce the rate at which it compounds. The full operational response, including contract structure and intake processes, deserves its own treatment, and I will write that one next, as I continue to experiment with solutions.The Deeper StoryWhat we are seeing in 2026 is a structural shift in the agency-client relationship that has not been named yet. The client did not stop being our client, nor did they believe they could actually build the full product on their own.The client added a third party to the relationship, one that operates with no context, no contract, no accountability, and no awareness that it has been added at all. And the client trusts the new party more than the old one, because the new party is faster, more agreeable, and more impressive on first read.This is the thing nobody saw coming about AI in client work. We are partnering with our clients' AI assistants. Assistants that do not have our context, cannot see our contracts, do not know what stage the build is in, and were never in the discovery sessions where the actual product was defined. And the client trusts the AI more than they trust us, because the AI never says no.The path forward is not to compete with the AI on speed or agreeableness. The team will lose that contest every time. The path forward is to make the team's context visible, treat the AI as the stakeholder it actually is, and anchor every conversation on the outcome the product is supposed to produce, not on the artifact someone generated last weekend.If you are seeing this pattern with your own clients, you are not imagining it. It is real, it is structural, and it is getting more common every quarter. Name it. Talk about it with other product leaders. Share what is working and what is not. The agencies that figure this out first are going to have a real advantage over the ones still treating AI-generated documentation as if it came from an author.
